New Patient Registration
Before making an appointment, you must be registered with Gade Surgery. You can register by calling at the practice at any time during normal opening hours. When registering, please provide either your NHS Card or other proof of identity and address in the practice catchment area.
Full eligibility criteria including for overseas visitors can be viewed by following this link. The NHS registration form can be downloaded from here but you will need to attend the surgery with identification documents in order to complete your registration. If you are registering a child aged under 5, please also bring their vaccination record so that a copy may be taken to update our records. You will be asked to fill in a new patient questionnaire which can be downloaded from here or for a child from here. Also as part of our alcohol awareness campaign and what constitutes safe drinking please return our brief alcohol questionnaire which can be found by clicking here
Patients registering with Gade Surgery are allocated to a named doctor who is responsible for their care. You will be encouraged to see your registered doctor for routine consultations but consultations with other doctors would be possible at your request and subject to availability of appointments.
You will be invited to attend a new patient medical with a Practice Nurse.

We have a personal list system as we feel that this offers the best service to our patients. However we know this relationship may not always work and that patients not unreasonably want to change doctors. More information about this can be found here.
