We have now opened our registration books to existing and prospective patients that are moving or coming from an out of area (OOA) location. See home page for more details.
In order to register with Gade Surgery as a new patient (including OOA) please follow the below steps:
- Complete online Adult or Child Registration form – 1 form per patient.
- Upload relevant documents (ID/proof of address) – please refer to our registration policy for suitable forms of ID.
- We will contact you to confirm your registration. There is no need for you to attend the surgery to register with us. This will be done completely remotely.
- Please ensure all details are accurately completed before submission. This will reduce any delays in processing your registration.
- There is no need to contact the surgery to chase your registration status. We will notify you automatically in writing to let you know this is complete.
Proof of ID
We strongly encourage patients to provide ID as it will help speed up the registration process and gain you access to the patient access app. The app allows you to book appointments online/request prescriptions and view your COVID vaccination records.
How long after I submit my application will it take to be registered?
We aim to process all applications ASAP. We have a highly experienced team of registration administrators who work regularly to ensure your application is processed in a timely way.
As some registrations can be more complicated we advice all new prospective patients to not chase within 28 days from submitting an application.
The majority of applications are typically processed much faster than this.
What if I need urgent on the day care but I am not officially registered yet?
This is not a problem. Please contact reception via usual methods and we can temporarily register you to provide same day emergency care with a GP.
Can I register if I live Out of Area (OOA)?
Yes! Please see our home page for more details.
We review OOA applications on a case by case basis and reserve the right to not accept applications if not felt to be appropriate. We strongly encourage family members to register as a household. However, please note if living OOA we are unable to accept applications for children under 5 years of age due to lack of access to some healthcare information for children in this age group.
Named GP
Once registered with us, you will be allocated a named GP. You will however be able to see any clinician within the practice.

For more information about NCRS please see: National Care Records Service
Information for patients who wish to opt out of having a summary care record, including an opt out form is available here: Opting out
