Register with Gade Surgery as a New Patient

Registration process (including out of area patients):

  1. Follow the link below to register via NHS online services.
  2. Complete online Adult or Child Registration form – 1 form per patient
  3. We will contact you to confirm your registration. There is no need for you to attend the surgery to register with us. This will be done completely remotely.
  4. Please ensure all details are accurately completed before submission.
  5. There is no need to contact the surgery to chase your registration status. We will notify you automatically in writing to let you know this is complete.

Register online

We are using an NHS online service called Register with a GP surgery that makes it easy to register with this GP surgery. This secure system will email us your registration form.

Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.

The service is designed and run by the NHS, so your personal information is safe. It reduces our administrative workload and makes it easier for you to register.

Paper forms are still available if you need one.

Proof of ID

We strongly encourage patients to provide ID as it will help speed up the registration process and gain you access to the patient access app. The app allows you to book appointments online/request prescriptions and view your COVID vaccination records.

How long after I submit my application will it take to be registered?

We aim to process all applications ASAP. We have a highly experienced team of registration administrators who work regularly to ensure your application is processed in a timely way.

As some registrations can be more complicated we advice all new prospective patients to not chase within 28 days from submitting an application.

The majority of applications are typically processed much faster than this.

What if I need urgent on the day care but I am not officially registered yet?

This is not a problem. Please contact reception via usual methods and we can temporarily register you to provide same day emergency care with a GP.

Can I register if I live Out of Area (OOA)?

Yes! Please see our home page for more details.

We review OOA applications on a case by case basis and reserve the right to not accept applications if not felt to be appropriate. We strongly encourage family members to register as a household. However, please note if living OOA we are unable to accept applications for children under 5 years of age due to lack of access to some healthcare information for children in this age group.

Named GP

Once registered with us, you will be allocated a named GP. You will however be able to see any clinician within the practice.

Your Previous Medical Records

Please note that your medical records may take several weeks to arrive at the Practice.

If you are a new patient and require repeat prescriptions. Please organise an appointment with a clinician to discuss.

Your NHS number is a 10 digit number, like 485 777 3456. You do not need to know your NHS number to use NHS services, but it can be useful to have it.

When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.

The National Care Record Service (NCRS)

The National Care Record Service (NCRS) is the way in which health professionals now access the Summary Care Record (SCR). The SCR has not changed, it is just the way SCRs are accessed that has changed.

Organ Donation
