Before making an appointment, you must be registered with Gade Surgery. You can register by calling at the practice at any time during normal opening hours. When registering, please provide either your NHS Card or other proof of identity and address in the practice catchment area.

You will be asked to fill in a new patient questionnaire which can be downloaded from here or for a child from here. Full eligibility criteria including for overseas visitors can be viewed by following this link. You will need to attend the surgery with identification documents in order to complete your registration, and if you are registering a child aged under 5, please also bring their vaccination record so that a copy may be taken to update our records.

Patients registering with Gade Surgery are allocated to a named doctor who is responsible for their care. You will be encouraged to see your registered doctor for routine consultations but consultations with other doctors would be possible at your request and subject to availability of appointments.

You will be invited to attend a new patient medical with a Practice Nurse.

The practice does not discriminate on the grounds of race, gender, class, age, religion, sexual orientation or appearance, disability or medical condition.

Changing Doctors

We have a personal list system as we feel that this offers the best service to our patients. However we know this relationship may not always work and that patients not unreasonably want to change doctors. More information about this can be found here.

Guide to GP Services

The Royal College of General Practitioners has produced a useful guide for patients about the services on offer at GP Surgeries and how to access them.  You can download the guide below.
